02The business in one line

What does UnitedHealth do?

04Why it makes money

Why does UnitedHealth make money?

UnitedHealth keeps a thin slice of $447.6 billion of yearly revenue, so a small change in members' medical costs moves profit a lot.

In FY2025, medical costs took $70.16 of every $100 that came in. The medical care ratio (medical costs as a share of premiums) rose from 83.2% in FY2023 to 89.1% in FY2025. One point of that ratio on FY2025 premiums of $352.2 billion is about $3.5 billion (Cityfort calculation from the FY2025 10-K).

FY2021 to FY2025

  1. Employers, people, states and CMS pay premiums, set a year ahead
  2. UnitedHealthcare pays members' medical and drug bills
  3. Optum Health, Optum Rx and Optum Insight earn fees for care, drugs and services, much of it from UnitedHealthcare
  4. Medical costs against premiums decide most of the margin
  5. Outside Optum customers add revenue on the same systems
  6. What is left pays interest, dividends, buybacks and acquisitions
